Art. 18 - Law No. 13.709/2018 - General Data Protection Law (LGPD)

General Guidelines

We provide this form to facilitate the exercise of your rights provided in art. 18 of Law no.
13.709/2018 - General Data Protection Law (LGPD).
The use of this form is optional, but it is an efficient and secure way to process your request.
We kindly ask that, after completing the form, it be forwarded, together with
copy of the applicant'sidentity document and a photograph of the applicant holding the identity document next to his or her face, as well as (if applicable), copy of the power of attorney or document showing the holder's powers of legal representation, to the e-mail encarregado@tracado.com.br.
If the proofs of identity are not attached to this form, Traçado may request
additional information from the holder of personal data, in order to be able to meet this demand and avoid
any type of information security incident that could harm the applicant.

    Rights I Wish to Exercise:

    It allows you to find out whether Traçado has carried out any processing of your personal data, even if it is not clear whether you have had any previous relationship with the company.

    Art.18, I, LGPD

    This allows you to find out about the processing of personal data carried out by Traçado. This item should only be ticked if you have had or still have any relationship with Traçado.

    Art.18, II, LGPD

    It allows the rectification, in general, of the applicant's personal data that has been processed by Traçado.

    Art.18, III, LGPD

    Allows for the anonymisation, blocking or deletion of unnecessary, excessive data or data treated in non-compliance with the LGPD by Traçado.

    Art.18, IV, LGPD

    It allows personal data to be obtained in a structured manner that enables it to be transmitted to another controller by Traçado.

    Art.18, V, LGPD

    It allows the deletion of personal data processed with the consent of the data subject, after revocation.

    Art.18, VI, LGPD

    It allows you to obtain information about the sharing of your personal data with third parties by Traçado.

    Art.18, VII, LGPD

    It allows the revocation of consent given at a point in time prior to Traçado in relation to the processing of personal data.

    To assist Traçado in responding to your enquiry quickly and efficiently, we kindly ask that you provide as much detail of the information requested as possible. If possible, we would like you to provide us with specific times, times, dates, names, types of documents or any other information about the enquiry.

    Remember that Traçado may contact the applicant or their proxy to request further information if the application is not clear enough.

    Proof of Identity of the Data Subject

    Please attach to the application the proof of identity of the holder of personal data. The ID, passport, professional card or any other documents that contain the applicant's qualification and photo will be accepted as civilian identity documents. Together with the document, submit a photo of the applicant holding the document next to the face, to check the effective identity of the petitioner, as well as, if applicable, a copy of the proxy document (Power of Attorney, Court Decision or Identity Card of the holder, in the case of a parent of the holder).

    If proof of identity is not correctly attached to this form, Traçado may request additional information from the data subject in order to meet the request.

    By signing this document, the applicant declares, under the penalties of art. 299 of the Penal Code, that the documents attached are authentic and match the originals.
